Justification

Savella affects some chemical substances in the human brain known as neurotransmitters. An abnormality in these neurotransmitters is believed to be the main cause of chronic pain in the joints and muscles. Although the drug is not the best opti…………………..ults to resolve nerve pain resulting from shingles (herpes zoster) or herpes virus (Zimmerman, Astifidis & Katz, 2015).

Expectations

In the first week, I expected the client report reduced pain levels. Savella was also expecte………………………..ysical activities.

Deviations

On the positive side, the patient reported reduced pain during the day. This was confirmed by the fact that h……………….ddress the problem completely.

Decision 2

Given that the patient’s progress was recommendable, I decided to continue with current medicatio…………….wered the dosage to 25 mg after every 12 hours.

Justification

Considering that the patient was not experiencing pain during the day, my conclusion was that Savella was working and therefore it was unnecessary to discount the medication. It was also my considered opinion that a lower dose of the same would be appropriate in order to minimize the possibility of triggering Savella’s side effects such as breathing problems and hives. Moreover, Lyrica and Zoloft we…………………..e there is need to enhance patient’s appetite, mood, sleep, and energy level and may assist restore client’s interest in daily living. From the look of things, Zoloft could not address the specific symptoms reported by the patient in any way, shape, or form.

Expectations

I expected the pain to go away completely.

Deviations

Contrary to my expectations, the client returns four weeks later using crutches. He reports that his current pain level is 7 out of 10. Clearly, his condition is worse than the p……………………rtainly, the previous prescription was not effective.

Decision 3

Given the deteriorating pain levels, I decided to change Savella prescription to 2………………………..g orally at bedtime.

Justification

First of all, Savella is a Serotonin and norepinephrine reuptake inhibitors (SNRIs) that also possesses N-methyl-D-aspartate antagonist activity that produce analgesia at the point of nerve endings. Reducing Savella helps in managing side effects but d……………………exa inhibit the reuptake of serotonin and may consequently cause serotonin toxicity (Howe, Stephens, Ratzliff, Unutzer, Katon & Stephens, 2016).

It is important to note that the patient has complex neuropathic pain syndrome that may never respond to pain relievers satisfactorily. Once that fact is appreciated, the next step enta………………….plex treatment that include medications, physical therapy, heat and massage therapy and chiropractic care (Schjøtt & Bergman, 2014)

First and foremost, from an ethical point of view, the patient should be informed about the side effect……………..atient is also part of my ethical responsibility.
